Please request refills through your pharmacy first. Medication changes may require an office visit. Refill requests take up to 48 hours to process.

Authorization form to share your patient records with other parties.

Confidential Document Upload
Use the secure messaging feature in the patient portal to submit documents to our office. Alternate form if you don't have portal access.
Appointment Reminders
Our office uses an automated reminder system:
- Email reminder 3 days before your appointment
- SMS text and phone calls 2 days prior
Emails from the patient portal come from no-reply@valant.io - add this to your contacts to avoid spam filters.
We cannot guarantee reminders will be received. Upcoming appointments are listed in the patient portal. Please note your appointment times independently.
